Insert Controls menu options is grayed out

I received a file from a coworker with the insert controls icon on the ribbon grayed out. How do I reactivate it? I can see the insert controls icon in my other excel files. The file was originally an xlsx file for which I did a save as .xlsm.
I am able to create and edit macros. I just cannot create the control buttons to launch them.
 
Thanks, reason #4 was the answer. "Excel Insert button grayed out due to Display Options for the Workbook is Disabled." I would of never guessed that something under "For objects show all." given that it was right after group dates in the autofilter.
