Inputing criteria for a query

i have a table. one of the fields is database_name. I want a query/macro to give me all the data records from the table where database_name= the database name i will be propted to enter

SELECT Table1.Database_Name, Table1.Value
FROM Table1
WHERE (((Table1.Database_Name)=[Database Name]));

The above assumes your table is called Table1

It fires an inputbox that asks for a database name before running the query.
