INDEX MATCH Function help

I am creating a resource planning document and am using the INDEX MATCH function to pull through the rate from 'Sheet2' to the corresponding role in 'Sheet1'. For example Client 1 has an MD rate of 175 however this is pulling through as 359 and instead the 175 is pulling through against Client 3. I can't figure out what's gone wrong so would appreciate help. The link to the document is below

 

Excel Facts

Did you know Excel offers Filter by Selection?
Add the AutoFilter icon to the Quick Access Toolbar. Select a cell containing Apple, click AutoFilter, and you will get all rows with Apple
You are using different ranges for the index & the 2nd match function, so everything is offset, it should be
Excel Formula:
=INDEX(Sheet2!$D$2:$V$18,MATCH(F$12,Sheet2!$A$2:$A$18,0),MATCH($B13,Sheet2!$D$1:$V$1,0))

I know I am in minority but I always found the formula much easier to check if you start everything from the same corner of the data set.
