I am trying to do an INDEX-MATCH formula to pull the fields off my onboarding sheet to my dashboard sheet. I can’t figure out what I am doing incorrectly.

How to show all formulas in Excel?
Press Ctrl+` to show all formulas. Press it again to toggle back to numbers. The grave accent is often under the tilde on US keyboards.
I see nothing in that pictures. Please upload the file, in a file sharing and explain exactly what you want.

Assuming you want all the data, but just those columns:
Also assuming the OnBoardingSheet name is "OnBoardingSheet", update accordingly to fit your workbook sheet names.
(I added a column 10 for "EOD DATE")
Put this is cell A1 of your DASHBOARD SHEET:
Excel Formula:
=CHOOSECOLS(OnBoardingSheet!$A$1:$J$1,5,4,3,8,10)

What are you matching exactly? You have absolutely no data shown in those pictures. Except for some column headers, there's nothing to indicate what data you're trying to match.
If you had said something like "I'm trying to match employee name Tom Smith from Sheet A to Sheet B and have it return the location where he works" that would have made some sense.

Also rather than taking photographs with your phone, learn to take screenshots on your PC which is much easier and requires less steps.
