Importing to Microsoft Outlook

I am trying to import an Excel file of addresses and phone numbers to Microsoft Outlook. Every time the import wizards gets me to where I should be able to map the custom fields but It doesn't show me any of the fields or allow me to click next. Does the excel file have to have some kind of range specified? If so, how is that done?
