Im confused about connecting to database

I was recently asked to set up a database for my father. I made the database and we loaded all the information into it but now we are having some issues getting power pivot to pull from this database. It is a typical godaddy mysql server.

And yes I did call godaddy and make sure that remote access was allowed. Im confused on how to get this to pull properly. let me know if anyone can help and walk me through what to put into each field on the power pivot wizard to get it to pull properly Server name And Database Name
