If selection in table, start user form in next row

I'm trying to create a user form that opens up any time a cell in a table (or anywhere in a specified range) is selected, that will input data from the user form to the end of the table. I would also like data not entered in the user form to be auto filled in the same row, such as the current date (which would not update after being entered).

I've searched around, and the few things I have found either didn't work or I couldn't figure out how to adapt them to my needs. Any help is greatly appreciated!

The newer CONCAT function can reference a range of cells. =CONCATENATE(A1,A2,A3,A4,A5) becomes =CONCAT(A1:A5)
Actually what I think I need to do is have it so that when any cell in the row directly below the table, and in columns A through D, is selected then the user form will open and the data will be entered into that row.

Sorry if this isn't the kind of thing that should go in a reply, but I couldn't figure out how to edit the original post.
