If you're looking for help with formulas you'll need to give us something to go on, as there's no indication from your post.

I've uploaded the report template in RangeToBBCode .

My Boss has been expecting me to come up with that report but to no avail. Now, he's becoming impatient with me.
Please, I need you to fine-tune the report for me with the right formulars to derive the right result.
