Hyperlink based on 2 cell values

I have a workbook with multiple sheets, on one sheet I have a column with sheet names and the second with cell address
example
A2 = CONTROL ROOM AE5 B2= $C$4
How can I create a hyperlink that goes to the value in A2 (sheet name) and B2 (cell).
