How to use an array in an IF(AND statement

I need a formula that will return the contents of a cell if two other cells contain one of the numbers in two different specified arrays. I laid out the argument in another question on this board but may not have done a very good job of if. It can be found here:

I need a cell in one tab to meet multiple criteria using an array found on another tab and then if true place the value of that cell in the cell where the formula is found.
