how to separate this into columns?

This information was in columns in a PDF. When I try to paste it into Excel it goes into one big column. How would you separate this?

This information below should be 5 rows, 10 columns. The last column is email address, which in 4 out of 5 cases is empty.

The first row should read like this (I put a / where there should be a space here) :

GP Psychotherapy / John / Grisham187 Queenston St. / / St. Catherines / L2R 3A3 / 905-688-5125 / 905-688-9648 / /

If this is the ONLY data then you can just separate it using the excel's built-in "text to columns" feature in "Data" tab of excel program. Then later you can use any type of string concatenation operators. VBA will be an overkill here but let me know what is the volume of task at hand.
