How to create scatter plot with activity names (as y-axis) and 3 sets of dates (as x-axis)

Hi everyone,

I've got a data set as per below:

Activity
Original BL
Current BL
Forecast/Actual
Activity 17/09/2018
16/08/2019
30/08/2019
Activity 226/10/2018
30/08/2019
14/09/2019
Activity 31/11/2018
19/09/2019
17/10/2019
Activity 48/01/2019
30/09/2019
21/10/2019
Activity 523/01/2019
31/10/2019

I want to represent this data in a scatter plot with the Activity on the y-axis (i.e. Activity 1, Activity 2, etc) and the corresponding dates on the x-axis (i.e. Original BL, Current BL, etc).

Looking for help to do this. Thank you :)
