how to creat webpage froms in notepad with submit button

i need to creat a webpage froms in html and send that link to users. once the user click on submit the forms detail should be send via mail to me. Thank you!

Sorry, but that's rather vague.

How much HTML do you know?
Have you looked at the tutorials and resources in places like sitepoint and w3schools?
Will this be part of a website?
