How do i create a form to give me query results

Instead of using a parameter query, I would like text boxes added to a form. Once they've been filled in need to run the query.

Can anyone help please?

you can use the Builder in the query to reference your text boxes. The results should be something like
[Forms]![YourForm]![YourTextField]
