How can I merge a list of names from excel into an excel form?

I have a list of names on one sheet and a form on another sheet (same file). I want to auto fill the name from the list I have but create a separate form for each name. I know you can merge your excel list into a word letter but the form I have needs to be in excel. Can someone help me with this task?
