i need help creating a Java program to capture paiient's information and format it to display to console as a proof of concept.

I'm afraid this isn't really a java discussion forum. Many questions here - where is the data stored? How will it be retrieved? What is the purpose of the display? What kind of business logic is to be implemented? Will you need a GUI for your end users? Rest assured that Java can output information to the console. That is really only the tip of the iceberg. You may want to talk to your IT department or a java consulting firm.
