Help creating multiple lines

I would like to create an order sheet that expands into additional rows as below. I don't want to use VBA but happy to add to my limited power query knowledge

For example:
Product 1 is "ladies summer uniform" consisting of skirt, t-shirt, hat

Product 2 is "mens summer uniform" consisting of shorts, polo shirt, hat

Product 3 is "winter extras" consisting of long pants, jumper.

I would like customers to be able to complete a form to select a specific product with their details ( name, contact number, address) and a sheet created with

LINE 1: Order 1 Name. Address. Contact number. Skirt
LINE 2: Order 1. Name. Address. Contact number. T-shirt
LINE 3: Order 1. Name. Address. Contact number. Hat
LINE 4: Order 2. Name. Address. Contact number. Long pants
LINE 5: Order 2. Name. Address. Contact number. Jumper

Is this possible?
