In the excel spreadsheet cell reference "A2" against this we are monitoring others cells like B2:T2, If the targeted cell reference value (A2) is not matching particular cell between (B2:T2) automatically form (pop-up screen)will be generate reason for not meeting value below standard questions will be raised.

1. what is the action plan taken
2. what is the timeline for closure
3.Root Cause

Did you want this form to pop up if one of those values changes? If we created a form to ask these questions, where would you store them and how would you see/review the information later?

Would it be better to have 3 new columns next to the value to put the Action Plan, Timeline, and Root Cause. You could use conditional formatting to highlight the need to complete those if the value meets the criteria.
