I am sorry if this has already been answered, I am not sure what to search for.

I am trying to do something like a VLOOKUP, take a value to find and return data from a table.
The problem is the initial lookup value will show up multiple times in the search table and I will need to choose between those values.

Can anyone please help me create a formula for this?

For some reason, that formula is not returning any results at all.
