Formula to Show Harvey Balls

So, I am being asked to put together from percentages a harvey ball presentation. What I am needing is an If statement(I would think) but I do not know how to call in Harvey Balls that way using conditional formating.

Here are the ranges; Green 95 - 100%, Yellow 90 - 94%, Red 89 - down.

Move date out one month or year
Use =EDATE(A2,1) for one month later. Use EDATE(A2,12) for one year later.
I have, it really doesn't fit into what I am hoping to accomplish. You see these will be tied into graphs that are updated regularly. Thank you.
