Form Question

I created a form to collect data, but it shows in Design view, but not in Form view: See images

Confusing. You show form view in the first post but claim it only opens in design view, which doesn't look at all like the first post.
I suspect your problem is that the form view shows no controls. If that is the case, see if any of these causes apply:
