Form - multiple cells in one entry

I have a sheet with a 1st column "supermarket", a 2nd one "type of product", a 3rd "product"and a last one "price". There is one row per product.
I would like to make this sheet user friendly and create a form. But I would like the user to be able to, in the form, choose (just once) the name of the supermarket, define the corresponding sold products and prices and add the record to the data sheet.
Is this possible?
I'm working on Excel 2011 for Mac.
