Floating Task Bar Items

Crap, I said Task Bar in the subject, but I really meant Quick Access Tool Bar.

On Excel 2016, I have shortcut icons all along the Quick Access Toolbar. I have them sequenced from left to right in an order that makes them most usable for me.

Lately, a couple of the icons have started floating around For instance, I keep the "File Open" icon in the third spot from the left. But out of nowhere and for no reason, it will appear all the way over on the 13th spot from the right.

I put it back where it belongs, then a day or so latter, it floats back to the other spot.

What's causing that??
