Extracting Data from Table to New Data Table

Dear All

I have attachment workBook
IT has two sheets
1st: Data OT, this has all patients details
2nd: Laboratory results of HCG testing

At last Sheet (HCG) we adding the results of blood testing, according the received patient
for that we are writing the Patient file ID at column A
and blood result at Column B

What I need is to:

Transfer the result from table at HCG file to patient ID corresponding cell at Data OT sheet.

I did this equation
it gives part of data for only one ID:4165

but it missing some readings
and not working with other patients.
