Excel Database will not open.

I created an Excel Database using the Template Wizard. It worked for awhile, but now when I fill in the form and click on save I get a message that the connection to the database cannot be made. I have had problems with this database from the beginning, and re-did it several times.I thought I had it working.An earlier database I created in the same way continues to work fine. Any thoughts on what is going on?

The =SQRT(25) is a square root. For a cube root, use =125^(1/3). For a fourth root, use =625^(1/4).
This is a reply to my own message. I hope that is okay. I had some time so I went back in and realized the database and the form had one word difference in the names. I deleted and re-did them making sure everthing was the same. Seems to be working now.
