Excel/CSV to a formatted form

I know this might be in the wrong place, but I don't know where this would fit. LSS I need to get a list of data that is exported to an excel sheet into a certain format that can be printed to a PDF. The data being returned would be Name, Address, Email, and a short response. Then i need that data to be exported to a PDF in a certain Format. (See Attached) any help would be extremely useful.

What is the format of the raw data that you are starting with? It is probably easy to reformat using formulas, but we need both input and output.

There is no "short response" in your example. Can you provide as realistic an example as possible for what you want your output to look like?

Data Flow Process..... User enters data into form during an event (Name, Address, Email, Short Message), After event, data is exported to either an XLS or CSV file with the appropriate column headers and all user data in below cells. Then that data in the excel table would need to be pushed into a PDF with a certain page formatting. The attached image on the original post would be the ideal format except adding a field under the address for the short message. That PDF will then be sent to the event owner so that he can view all attendees.

The appearance of the form is not relevant.
data is exported to either an XLS or CSV file with the appropriate column headers and all user data in below cells.
What is needed is the format in the file after the data from the from is exported.
