E-mail attachment corruption - CDB6F.XLS

I work developing and managing databases and spreadsheets for a company that uses Windows NT, Lotus Notes e-mail and Microsoft Access. We experience a problem which occurs without warning periodically to Access users and no-one in our IT Department has any idea what causes it. Briefly when trying to send a report, snapshot etc as an e-mail attachment the file name suddenly becomes converted to a spurious name always in the format above. The item being sent is converted into excel whatever the format originally. The problem cannot be resolved unless the hard drive of the affected PC is reformatted and the relevant software reinstalled.
The problem only ever has affected one report in any given database at any one time.

Any ideas for a solution as reformatting a hard drive and reloading the software takes anything up to three or four hours which des not make me popular and people think the problem is me!!!.
