I am stuck with an excel issue, where I need to look values based on

The =SQRT(25) is a square root. For a cube root, use =125^(1/3). For a fourth root, use =625^(1/4).
Hi,
create a column in your db after column D with this formula:
= B2&C2
and then drag it down

and then in your performance type sheet you can use this formula for needed data:

= VLOOKUP(A3&B3,db!$E:$H, 2,0)


db is your DB sheet name

I got it.
use this formula in array mode for performance type column:

=INDEX(db!E:E,MAX(ROW(db!E:E)*(db!B:B=A3)*(db!C:C<=B3)*(db!D:D>=B3)))
