Item = DLookup("item", "master", "vocab='" & vocab & "'") if it is blank, then look the value in the next column "Itemsecondary description"

Quite easily. One approach that should work:

SQL:
DLookup("Nz([item],[Item secondary description])", "master", "vocab='" & vocab & "'")
