Difficulty Creating Lookup/Match Formula

Hello,

I have one table that has the following column heads
and below what is in each column:


Title Day Difference 1 Week 30 Days 40 Days 70 Days
Text Number True/False True/False True/False True/False


In a separate sheet, I have this table:

Title 1 Week 30 Days 40 Days 70 Days



I am having a problem creating a lookup/match function that will scan the 1 week/30/40/70 day columns for any values with True, and then putting those titles that match in the separate sheet.

For example: A Lookup that looks for TRUE in the 40 days column in the first sheet will write all the Titles in the second sheet.

Does anyone have any suggestions?

Can you provide some sample data with expected results?
