Dialogue boxes

I created a database and then imported data from excel.
After I imported from excel when I try and open a form I get numerous boxes asking me to input and ID no, a surname etc. i.e. each of the field headings which are in the one table I have.

Why have they appeared? If I click OK on each of them as they appear and type nothing eventually my form will open.

How do I get rid of them.

Did you create this form?

Is it based on the table you imported?

Are the ControlSource's of the controls on the form different to those in the table?

Hi,

I created the form and table (as a blank table)
I then copied the data into the table from an excel spreadsheet and the control source's in the form are the ones in the table I created.
