Dialog in Word

In Word, how do I launch a dialog when doing a File > New (custom template), that asks for a name (Last Name) and then both uses that name in the salutation; and immediately does a File > Save As using the name and 6-digit date, e.g. Jones 071514.

Also, I'd like to have a 2nd dialog at the beginning that asks for a number, which when input pastes a specific version of a paragraph in a specific place.
