Database, hyperlink and copying

I have a workbook containing 2 worksheets. One is the database, and the other is a form. When filling in the database it populates the form. No biggy.

My question is, I have 4000 rows of data and wish to copy the form for each row. make the forms absolute, hide the forms and hyperlink. Any help appreciated.
