Data Validation Lists with Multiple Contingent Stipulations

Hello,

I am trying to create this spreadsheet and am stuck with multiple conditions in a data validation list. The Excel attachment is pretty self-explanatory. I am unsure of how to implement this correctly.

-Edit - Forgot to mention that column's with Green in Rows 1/2 are done. Its only columns F-I row 6 that is the work area. Row 1/2 Are explanations of how i'm trying to create the table.
