customizing email collection form

I'm sending out a form to users from Access 2007 using External Data -> Collect Data -> Create Email.

This process creates an ugly form with a bunch of filler text that I don't like. I don't see anywhere to change or delete this text, or change the default email form. All I can do is change the name and type of each field in the database for which I'm collecting data.

Also, it would be great if there was a way to collect data through a list box or some other type of restrictive mechanism.

I realize that these things can be done using InfoPath, but neither I nor the email recipients have InfoPath. There's nothing I can do about this. I have to use HTML.

Thoughts?

I just made the form and the database but had no idea this form was so plain.
I've been searching for a while today and came up with limited topics.
No infopath here to use.

No, nobody ever responded, and I never found an answer. I think the sad truth is that there isn't a way to do this without Infopath. I suppose that a custom VBA solution is also possible, but I don't have the skills to build it.
