The short answer to your question is "it depends" - if your data structures lend themselves to the task and provided you want to limit the number of weeks shown, then the answer is "yes - it should be able to be done".
Before we can accurately answer your question, or provide any assistance, there are a few questions that need to be answered.
Do you want the number of columns on the form fixed (e.g. show only the last x weeks)? If so, how many weeks do you want on the form? Or are you wanting to show all weeks on the form?
What are the variable names you want on the form? Are these sourced from a query or a table?
Lastly, are you capturing the year and week number in your data? If so, are they in separate fields within the table? If so, how are they formatted? Or are the year and week together in one "date" field?
