Creating a list from Multiple tables

Hello,
I was after pulling a list with 3 Headings "Advisor" "Site" and "Date"
The list will be looking at multiple tables and advisors could work at 2 sites on the same day.

in the example i have put that advisor "Test 5" has been at Sites 1 and 3 on the same day.

is this possible at all? to show they have worked there is a 1 in the box.

i would like an entry on the list for every time there is a 1 in the box.

Hope that makes sense.
