Create Outlook Design form Office 2007 seen by customer

I am attempting to create an Outlook Design Form that I can use to ask some questions and send out, when I designed my form and published it then use it, then send it to myself as a test I only see the normal form beign returned which means my customers doesn't see the designed form I have created?

Does anyone have any good websites with some basic tutorials that will help me overcome this.

It is not so much the design but whey can I not see the designed Item when I send it.

I have a deadline looming.
