Create new user form for new entry

In a nutshell
Work Book; "Client Services Analysis"
As a new name is put in a column B: and there is a value in column J
Open a form
Add the name in Column B to a specific cell
Copy the form and rename it based on the contents of cell in column J
Put it in another directory and maintain hyperlinks back to Client Services Analysis


This is for a non-profit working in Crises Care for ICU patients families.

I need to be able to generate report forms for each new client.
I realize this is a bit complicated but would appreciate any help as we are not funded as yet.
