Create form in Outlook to sync data to excel 2013

I am new and not sure of what to do.

I want to create an address form in Outlook to send out and collect the info in excel. Just some emails not all emails going out. I could have a 100 addresses to collect.

Format cells as currency
Select range and press Ctrl+Shift+4 to format cells as currency. (Shift 4 is the $ sign).
