Create a Word form that returns an excel report

I would like to create a Word template (or 'form') for end users which would, in addition to acting like a form, allow for submitting two parameters that would generate a real time status report within the Word doc.

At the top of this form would be the two fields where a team lead can enter (1) the name of a project they’re working on eg. MobileAppABC and (2) the name of the current build 10.20.3333.5342.
Upon hitting <enter> those two parameters are passed from Word to a TSQL query which generates a report and chart in powerpivot Excel format that is passed back and rendered from within the Word document.

The end user would also be able to edit certain areas of the form with free style typing ie. summary comments about his project.

Does anyone have experience with creating such an integrated solution?
