Create a survey with a customised report

Hi - I need help!

I am trying to create a survey with multiple choice answers, after which completed, will give the respondent a score and report based on their answers. I have tried to do this in Microsoft forms with the questionnaire feature, but I need to weight the different answers within a question and this feature does not exist. So I have resorted to using a normal excel spreadsheet which lets me do the formulas I want to weight the answers and create the report. My problem is - how do I receive everyone’s responses if I am just creating the survey in excel? I want to create an aggregated view of all responses as well as them receiving their individual report.

Any help is greatly appreciated - it’s driving me crazy!

Excel Facts

Remove leading & trailing spaces
Save as CSV to remove all leading and trailing spaces. It is faster than using TRIM().
