Convert Check box tick to text

I have created a series of check boxes that put -1 into fields check1 ,check2 etc.

How can I convert the -1 data to read as a text line in a report?

What text would you like instead of "-1" ? Where is the checkbox located ... in a userform or on the sheet ?

The check boxes are on a form.

the text I want is

Piano level 4 Check box 1
Piano level 5 Check box 2
Piano level 6 Check box 3
Piano level 7 Check box 4

There will be others but I should be able to follow an example.

The text is intended to output to a report and there may be two or more selected.
