This is my raw data:

https://drive.google.com/file/d/0B_WMlJ39MlmaUXhENnFlNDVLeGc/edit?usp=sharing

I want to get to something like this:

https://drive.google.com/file/d/0B_WMlJ39MlmaNG1vZ0ttdnVFUlk/edit?usp=sharing

How do I do it? I cannot get the formula to repeat. I'm using the following formulas to do this:

https://drive.google.com/file/d/0B_WMlJ39MlmaTURvTHQ2X0M2Y1k/edit?usp=sharing

Is there an easier way? Right now the I manually entered sets of four from rows 2 to 5. When I try to autofill, for row 6 onwards it does not work. Let me know if you want the original file and I can email it to you.

Thanks in advance -any help will be appreciated.
