CONCATENATE BIO, CLOTHING SIZE, SHOE SIZE, WISH LIST ETC. IN 1 CELL PER APPLICANT

I need to create a formula that will populate the field i will use (column h). When done correctly, it will look like this (with corresponding cell content):

Referral type:
Family background:

Adult #1
Age:
Gender:
Clothing size:
Shoe size:
Wish list:

Adult #2:
Age:
Gender:
Clothing size:
Shoe size:
Wish list:

Individual #1:
Age:
Gender:
Clothing size:
Shoe size:
Wish list:

Individual #2:
Age:
Gender:
Clothing size:
Shoe size:
Wish list:

The headers i wrote above are mandatory for each applicant (i.e. Referral type, family background, etc.). I had a stroke, paralyzed on my right, and it's too difficult for me to do. For years my non-profit adopts families for christmas. They sign up, and i combine all the necessary information on our internet portal, so that people in the community can adopt them for the holidays. Your assistance is greatly appreciated. Hopefully it is self-explanatory. Penny
