Combine & de-duplicate

Hi all - I have a worksheet of business names with contact information with multiple criteria. Many of these contacts (names) are duplicates, however their associated data may be different in some cells. My example below shows what I am trying to do. Is there a VBA I can run to accomplish this??

Any help is appreciated.
