Chart Help!

I was wondering if someone could help me!

I want to create a simple chart which will show a range (1 to 5) and then where four different values fall on this range. To complicate things slightly I want to put two boxes at either end of the range in which will be a descriptor of what 1 looks like and what 5 looks like (it's essentially a competency scale where 1 is a negative indicator and 2 is a positive indicator). How should I set out my data in order to create this chart (assuming this is possible!)?
