Built VBA userform in Windows - won't run in Excel for OSX

I have just made a simple userform in Excel 2010 on Windows for capturing information from a large number of users.

I wasn't aware of differences between userforms/VBA on Windows and OSX so the form obviously doesn't work on Mac - it asks for the macros to be enabled but then follows with:

This workbook contains content that isn't supported in this version of Excel

Did some reading on the internet and have seen people mention simply editing the code to fix it but I am not sure where to start. I tried going to the back end on Mac and can see the code but everything else is missing; no tool box, no userform and no way to run the code.

Any tips? The people I will be sending this form to will be a mixture of Windows and OSX users and I just need the form to work out of the box (or at least nothing more complicated than enabling macros). Thanks.
