Attendance (In and Out) Capturing Using Excel Form

Can you guys please help me with coming up with an Excel Form that captures logins and logouts with the following features:
1. To login and catch timestamp, the form will ask for a username and password (other stuff will also do, like just simply typing in employee ID, scanning QR code, etc.)
2. Must always follow log sequence, like for a certain day they must not be allowed to log out without logging in first.
3. Has manual override function only accessible to authorized user.
4. Has approval function for leaves, undertime, etc.

It doesn't have to be fancy - a simple form with the most basic buttons for attendance logging and manual editing feature is fine.
 

Excel Facts

Move date out one month or year
Use =EDATE(A2,1) for one month later. Use EDATE(A2,12) for one year later.
