Here is the page in question Dr Web Contact Form

I am stuck with how to put the text above the input box.
Currently its to the side.

Any advice that can be advised so i can simply edit my existing working code.

Excel Facts

Spell Check in Excel
Press F7 to start spell check in Excel. Be careful, by default, Excel does not check Capitalized Werds (whoops)
That looks like a JotForm contact form from the source code. Why not ask on the JotForm forums?

It has nothing to do with jot form.
This is a standard html table code

My form is HTML,that code is using css.

I need to just add the correct extra HTML code to my existing form.

Your thread title asked about CSS and HTML coding. The page to which I linked looks at the HTML and the associated CSS. Did you read it all or just glance at it and dismiss it? The point is that it's not really very hard to find out what you want to do via a Google search. :)
